How to Apply
Before completing the Application, you must answer yes to each question below:

1. Has your practice met the QOPI Certification scoring requirements? (check by viewing your QOPI Certification Report)
2. Did you choose all modules applicable to your practice?
3. Did you sample in accordance with the QOPI sampling methodology?
4. Can you verify that your practice is in compliance with all of the chemotherapy safety standards that are listed in the Site Assessment Questionnaire?

Log-in Help
Your log-in ID for the Certification application tool will be the same as your QOPI username. You will receive a password for the QOPI Certification application that is different from your QOPI password.
Password Retrieval Help
-Click on Apply Now button
-Click Password Help link
-For E-mail Address, enter the Corresponding Physician or Administrator’s e-mail address that is
registered for QOPI
-For Site ID, enter your QOPI site ID number
This will generate an e-mail that will include your new password for accessing the Certification application.
